    Incident Description: My son was sucking on his soothe pacifier and I looked over and he had gotten the entire pacifier in his mouth. He was silently gagging and his lips turned blue. I removed the pacifier from his mouth and he took a breath.

    Incident Description: My 6 month old daughter was laying down in her crib and had an AVENT Pacifier in her mouth and it seemed to have flipped and got stuck long ways in her mouth. She screamed and cried and bled from the pacifier cutting her gums and the roof of her mouth.
