    Incident Description: Ozark Trail Oversized Mesh Lounge Chair. I was sitting in chair when, with no warning, the front left rivet at the seat level broke causing the chair to collapse. I only received a minor cut to my left calf from the sharp rivet as the chair collapsed.

    Incident Description: Two year old healthy female playing with table unattended. Mother found child supine on top of the upturned table on the floor with neck between the folded table legs. Child asphyxiated and was pronounced dead at the home.
