11/1/2014 This date is an estimate
(02/17/2015) The consumer stated that he turned the light either on or off. He was not sure. There was a bang and the light exploded. The entire kitchen was covered with glass.

The bulb was in an overhead fixture in the middle of the kitchen.

No injuries.


(02/28/2015)The consumer contacted the company to advised them of the incident. The company advised the consumer that they had never heard of anything like that happening before. He was asked to send pictures of the incident. The consumer asked the company to remove the bulb from the fixture. He was advised that they could not because they were located in California and he was in New Jersey.

The consumer called Costco who offered to replace the product.

We spoke with the consumer named in this complaint on 2/23/15. He explained the subject light bulb was used in a recessed ceiling light fixture. It is possible this bulb was damaged during use or may have shorted out inside the socket. Based on the information provided, these would be the most likely reasons why the bulb failed. We are unable to examine the subject bulb and are therefore uncertain of the exact reason why it failed in the manner described. During our telephone call with the consumer, we explained that both our packaging and etch on the product caution not to use this bulb type in recessed fixtures. Instead, a reflector type bulb is best designed to perform in this application. We concluded the call by offering to send replacement energy saving reflector bulbs that are more suitable for recessed light fixtures.
