The Flushmate III Pressure-Assisted Flushing System in the American Standard toilet in my apartment unit exploded. It shot parts of the porcelain tank into hallway. Part of the tank compressor landed 3-4 feet away.

The toilet was not in use at the time, no one was in the bathroom or hallway, and no injuries were sustained. As we were present in an adjacent room, we were able to shut off the water almost immediately. Other than the shattered porcelain toilet tank and destroyed pressure unit, no property damage occurred. We needed to mop up the water and sweep up the porcelain shards. The apartment maintenance team replaced the toilet tank and pressure unit very soon afterward.

It appears the unit was one of those covered under the earlier product recall (i.e. recall number 14-089 dated January 23, 2014 or 12-201 dated June 21, 2012). Apartment building was built sometime between 2003 and 2004.

I've lived in my apartment since March 2005 and the toilet's pressure unit had not been replaced during my residency. There are roughly 100 units in my building and another 100 units in an adjacent building built a year prior to the building I live in. Some units have 2 bathrooms. Maintenance indicated they replaced a pressure unit in another apartment the week previous as the unit cracked, depressurized, and leaked water into the undamaged porcelain tank (i.e. that incident did not result in the porcelain tank rupturing). Maintenance was not aware of the product recall. I provided them the recall information.

Flushmate appreciates receiving notification of this report and the opportunity to provide a response to the Commission. After receiving notification of this incident from saferproducts.gov on July 1, 2015, Flushmate immediately called the consumer in order to provide a satisfactory response and to thank the consumer for the information.

Flushmate next contacted the property's maintenance supervisor to ensure that the reported incident had been resolved to the consumer’s satisfaction, to answer any questions about the Flushmate III recall, and to request a list of serial numbers from any other Flushmate-equipped toilets installed at the location.

With no prior warranty registrations or requests for warranty assistance from the location in Flushmate’s database, and no other prior contact from the maintenance department regarding the product, Flushmate had no prior knowledge that any Flushmate units were installed at this location. Flushmate was therefore unable to send a direct notification postcard following the announcement of the Flushmate III recall.
