  • Incident Description: Product: Summer Infant SwaddleMe Stage 2 Baby size small/medium. The swaddle pajamas were being used as intended with the correct size and age of a baby, and followed the directions of how to [fastener] them snugly on the baby. After using the swaddle for 4 weeks, our baby would get her hands and then arms out of the swaddle due to the material becoming stretched out from normal use. The hazard was due to when she would wiggle, the stretched out material/swaddle would wiggle up towards her head, giving her hands access to the opening of where the vertical leg sack material and the horizontal material parts meet. She would stick both of her hands out, then would pull her arms up towards her face in her sleep. I would find her like this asleep, and the horizontal material would be all pushed and squished up around her neck and partially or completely covering her mouth (still able to breathe out of her nose luckily). Thinking I was not swaddling her tight enough, I would continue to try using the product, but this same scenario happened 6 times in 1 month (even though I would [fastener] the swaddle so tight that there was no space between the swaddle and her skin, and she was unable to move when I first swaddled her until wiggling and stretching it out over a few hours and the material stretched and then moved). I stopped using the swaddle when one time I came in to check on my baby and the same scenario had happened again, but this time the material was pushed up over her mouth AND nose (still breathing when found her). The swaddle would never come undone and still was together, but would become stretched out and move as she slept, which allowed her hands to poke out of the opening (see photo) of where the corners of the material meet, causing a dangerous suffocation hazard.
