Consumer's 4 MOM son was swaddled and placed into bassinet on back with blankets present. At night he was found unswaddled, on stomach, not breathing. He was taken to a hospital and was pronounced. Bassinet was well used and had possibly a concaved bottom.

  • Incident Description: 4 month old child is placed in a swaddle on his back in the Ingenuity Dream & Grow Bassinet on August 31, 2019 around 10am. Blankets are present underneath the child and on top of the child. Around 2:30pm, mother finds child out of the swaddle, on his stomach in the bassinet not breathing. Child was transported to the hospital where he was pronounced deceased. Bassinet appears to be well used and that the bottom may be concaved.
