- Incident Description: While using the nested bean zen one swaddle last night, the material around the neck bunched up around her neck, while moving around during light sleep, causing her to have difficulty breathing causing a potential strangulation incident. The swaddle was promptly removed and my child is fine at this time. I followed the instructions for use. I had the arm zipper portions removed to have my child's arms swaddled out as we are planning to transition from swaddling soon. My daughter was wearing the correct size, 0-3m, for her weight and height. She was 10# 11oz and 23 inches long as of her last doctor appointment on 4/19/2023. The product fit her appropriately based on appearance as well.

- Nested Bean: The NB size chosen by the customer is correct for infant’s body weight. Babies may move during sleep, however the precautions taken throughout our design, manufacturing and safety testing process, given below, help ensure infant safety under unforeseen circumstances.
The internal arm band when fastened around infant’s torso, as per instructions, prevents the Zen One from rising towards mouth and nose. The neck circumference of Zen One NB is 9 – 9.5 inches, which is smaller than the 13.75 in – 16 in head circumference reported for the Preemie to 3-month age range as per ASTM standards. This helps the infant from sliding into the product during sleep. The cotton fabric and the weighted portion in the 0-3 month products have been tested for suffocation hazard and allow adequate air flow. Even though the Zen One has helped many of our customers, we continue to take feedback and improve our products.
