Incident Details

Incident Description: Purchased Halo Innovation Flex Bassinet. Upon assembly noted there to be a suspected tilt in the mattressnon the side that is not directly supported by the legs. On several occasions I have woken up to find my 3 week old with his face pushed against the mesh on the side of the bassinet that is not supported by the legs or any structure supports. Infant is always placed in Bassinet on his back and placed in the middle of the Bassinet on all occurances prior to finding infant appeared to have rolled to the mesh. Please note infant is unable to roll unassisted at 3 weeks of age. This evening I awoke to find infant rolled to stomach with head against mesh. Infant is unable to roll unassisted to stomach at 3 weeks of age. Highly suspect this product has a manufacturer flaw due to the lack of structural support causing the mattress to tilt when weight is placed in the Bassinet. This is a huge risk of SIDS and I do not feel this product supports or complies with the defiend safe sleep practices outlined by the CDC or APA. Please note infant well within defined criteria of use for this product as outlined using their manual supplied with the product. At time of occurrence infant 3 weeks of age and less than 9 lbs.
